Liability agreement

TERMS AND CONDITIONS

(Applied to Daycare and Overnight Guests)

Paws Inn “Plus”, Inc. Dog Daycare and Pet Lodging, it’s owner, employee(s), or any other person(s) affiliated with this company shall hereinafter be referred to as “Paws Inn “Plus” or Donna’s Paws Inn “Plus” Dog Daycare and Pet Lodging. The owner(s) or assigned person by the owner of the pet staying at Paws Inn “Plus” will be referred to as “client”.

1. I agree to hold harmless, Paws Inn “Plus”, Inc. dog daycare and pet lodging, and all their officers, employees and staff for any sickness, injury to, or loss or death of any pet left in their charge.
2. I shall indemnify Paws Inn “Plus” , Inc. against any claims made against the corporation or losses or damages of any kind suffered by Paws Inn “Plus”, as a result of my failure to inform Paws Inn “Plus” of any pre-existing condition the pet may have (such as illness or aggression problems). I understand and agree that in admitting my pet to Paws Inn “Plus” the facility has relied on my representation that my pet is in good health and has not harmed or shown aggressive or threatening behavior towards any person or any other pet.
3. I accept responsibility and liability for any and all harm or damages which my pet may cause or inflict against property, other pets, employees, customers, or any other people whilst my pet is in the charge or upon the premises of Paws Inn “Plus”.
4. Paws Inn “Plus” staff is hereby authorized to engage any veterinarian to treat my pet whenever they reasonably deem it necessary and I hereby guarantee payment of all veterinarian fees or charges incurred in this respect.
5. If my pet is not picked up by 5:45pm on the day of their stay, Paws Inn “Plus” will attempt to contact all phone numbers listed on the registration form. In the event they are unable to establish contact and schedule a pick up, Paws Inn “Plus” will either lodge my pet overnight (at the overnight rate) or charge an additional fee for being late. This cost will be immediately paid by the client.

I understand that when dogs play in groups, the dogs get rambunctious at times and nicks and scratches may occur. Daycare staff may or may not notify me immediately. If the injury is not serious, staff may feel it is ok to leave the dog until the end of the day and let me know about it when I pick up. If the injury is serious, I will be notified immediately at the phone numbers provided on the application. Paws Inn “Plus” will provide reasonable care and safety to prevent injury or illness for your pet(s). By signing this form, you acknowledge that you understand and accept the terms and conditions set forth by this agreement.
